Hours of Operation
Our hours of operations are Monday through Friday 8:00-5:00pm by appointment only.
· New Patients: Please hit “Book Appointment” above and then hit “Pre-Register” in the bottom right of the next page
· Established Patients: Please hit “Book Appointment” above and then sign into your Healow account on the next page. If you do not know your login credentials please contact our office directly via 508.898.0055 or info@communityneuroservices.com for assistance
Please notify us at least 24 hours in advance in the event that you will need to reschedule or cancel your upcoming appointment.
URGENT CALLS
After Hour/Urgent calls
· If you have an urgent matter and need to contact a physician during non-business hours you may do so by contacting our on call physician by calling our main line 508.898.0055. For any emergency related issues please dial 911 or go to your local emergency room.
Please remember to bring with you:
· Bring your insurance card.
· Bring a current list of medications.
· Bring a copy of any important prior records if you are transferring your care from another doctor.
· **Note that if a co-pay is required by your insurance company, it is due at the time of your visit and may be paid in cash, check or with a credit card.
